CareDome is a web-based care coordination platform built for teams managing remote patients. It brings readings, alerts, care plans, tasks and communication into one workspace, so clinicians can see what needs attention and move from data to action without switching between disconnected tools.

The challenge

Remote care teams were working across device dashboards, spreadsheets, inboxes and separate clinical systems. Patient data arrived, but the next action was not always clear. High-priority alerts competed with routine follow-ups, ownership was hard to track, and managers lacked one reliable view of programme performance. The platform also had to support care coordinators, nurses, clinicians and administrators without exposing more patient information than each role needed. Clinical usefulness, permissions, speed and audit history had to work together.

Clinical workflow mapping

We mapped the route from patient enrolment through incoming readings, risk detection, clinical review, intervention and follow-up. This clarified who owned each action before interface work began.

Product architecture

We organised the platform around patients, alerts, tasks, care plans and programmes. Role permissions, activity history and integration boundaries were defined at the same stage, so privacy requirements shaped the product rather than being added later.

UX and design system

We designed dense clinical screens around priority and next actions. Reusable tables, filters, status patterns, charts, drawers and forms gave the team a consistent system for new programmes and workflows.

Full-stack development

We built the web application, API layer and core data workflows, including background processing, notifications and integration-ready endpoints. The implementation kept clinical actions, ownership and audit history connected across the product.

Quality and release

We tested permissions, alert severity, time zones, missing readings, failed integrations, audit history and accessibility. End-to-end checks followed a reading from arrival through review and resolution before release.
